Treatment of pressure ulcers with free style local perforator flaps, in the General Hospital of Mexico

ABSTRACT
The treatment of choice for pressure ulcers has been musculocutaneous flaps. We propose the use of perforating flaps as a first option, or in patients with multiple previous surgeries and limited options for reconstruction. This study included 22 patients, 20 men and 2 women, aged between 18 and 65, with diagnosis of pressure sores treated with free design perforating flaps in the General Hospital of Mexico. The depth of the ulcers was grade III in 63.63% and grade IV in 31.81%. The number of identified perforating flaps was 6 in the sacral region, 3 in the trochanteric and 5 in the sciatic region. Hospital stay was 1 to 12 days. The complications were haematoma, seroma and dehiscence in 4.54% of the cases. Relapse of the ulcer was not present within three years of follow-up. The use of free design perforating flaps to close pressure ulcers is an excellent choice, provides well vascularized tissue, in sufficient quantity and it allows the design of skin islands that do not affect classic musculocutaneous flaps. The perforating flaps allow the reconstructive options to be extended and in cases with multiple previous surgeries offer simple options for complex ulcers.
