Robot-assisted and satelite teleguided laparoscopic cholecystectomies in Mexico

ABSTRACT
Introduction: With the satellite system for telehealthcare the (Telemedicine program) of the ISSSTE installed in 1994, we wanted to demonstrate the usefulness and feasibility of guiding or orienting laparoscopic surgical procedures by a robotic arm teleoperated (in the operating room) and at a distance via satellite to offer more precision and safety in laparoscopic surgical procedures and to diminish the index of morbidity and mortality as well as to carry out transoperative diagnosis on time and avoid unnecessary transportation of patients, which causes unnecessary cost to the patient, family, and health institutions. Material and methods: Two remote theaters were installed, one an operating room in the state of (Chiapas) and another working station (Mexico City). The working station was linked with telehealthcare centers of 19 Mexican federal states; all were linked by satellite. Surgical procedures were assisted by a robot inside the operating room; the surgeon received instructions from a expert laparoscopic surgeon from the remote tele-diagnosis center. Results: Two laparoscopic cholecystectomies were performed with no complications, assisted by a robot at the Belisario Domínguez Hospital in Chiapas on April 11 and 12, 2002, teleguided remotely by a surgeon via satellite localized at the 20th of November National Medical Center at a distance 1,277.8 Km. Communication was established by three lines: audio; videoconference, and the tele-operation of a videoconference camera. Delay of the information signal was 500 millise. Discussion: Results obtained only were possible thanks to the participation of the multi-institutional and multi-disciplinary program and projects including Robotic System Tonatiuh (Bioelectronics’ Laboratory of the CINVESTAV, Mexico City) and the Telehealthcare Program of the ISSSTE.REFERENCES
