Anterior tension-free inguino-femoral hernioplasty: HERD technique, preliminary report. Personal experience with 126 patients

ABSTRACT
Objective: To inform the results of the anterior tension-free inguino-femoral repair with mesh using the Hugo Enrique Reyes Devesa technique (HERD).
Material and methods: From January 20, 1998 to October 20, 2001, it was realized a prospective, longitudinal and descriptive study of 126 patients with inguinal hernia classified with the Gilbert-Rutkow/Robbins and Nyhus systems, operated with a modified Lichtenstein technique in which the mesh was fixed to the inguinal and Cooper ligaments.
Statistic analysis: Descriptive and frequencies.
Setting: Veracruz National Medical Center “Adolfo Ruiz Cortines” (IMSS).
Results: We studied 105 men and 21 women, aged from 16 to 92 years. The hernia was right sided in 63%, left in 32%, bilateral in 4% and femoral in 1%; primary in 92% and recurring in 8%. In 94% of the patients were discharged in 12 hours. Complications: wound infection (n = 1); vas deferens section (n = 2); vascular spermatic cord lesion and orchiectomy (n = 1); seroma (n = 3); edema of the spermatic cord (n = 4). There were not mortality or recurrences in the follow-up time.
Discussion: This is an easy to perform hernioplasty, allows the detection and treatment of associated or unnoticed femoral hernias; gives reinforcement of internal ring, the inguinal floor and femoral area, protecting the three possible areas of hernias (indirect, direct and femoral). The low morbidity is not related to the modification of the technique.
