Primary vs delayed closure in complicated apendicitis

ABSTRACT
Introduction: The diagnosis of appendicitis is difficult to date because of medications at the beginning of the clinical course, in addition to other causes. When perforated appendicitis is present, surgeons need to decide on the best treatment for the surgical wound to obtain patient recuperation as soon as possible.
Objective: Our objective was to determine whether surgical wound infection due to complicated appendicitis is lesser in patients undergoing primary closure than in those with delayed closure.
Material and methods: A prospective and comparative study was made. A total of 30 patient of both genders with a diagnosis of acute perforated appendicitis were studied; they were distributed into two groups of 15 patients, each. Group II patients were managed with delayed closure of surgical wound. Infection of surgical wound, premedications, complications, hospital stay, and leukocyte count were assessed. Statistical analysis was done with chi square, Fisher exact test, and Student t.
Results: Wound infection did not have a significant result in both groups; in group I, 6% of patients presented infection of the surgical wound and in group II, 20% (p › 0.05). In group I, hospital-stay was significantly less than in group II (p ‹ 0.05).
In this study, primary closure of the skin after appendectomy for perforated appendicitis was not associated with increased infection risk of skin compared to delayed closure.
