Comparación vs midazolam y dexmetomidina en sedación intraoperatoria para extracción de catarata con anestesia local

ABSTRACT
Objective. To compare, respiratory depression, hipoxhaemia and homodynamic stability of a geriatric population carried to cataract surgery with local anesthesia and sedation using Dexmedetomidine 0.5 µg/ kg versus Midazolam a 150 µg/ kg.
Material and methods. We studied 23 patients with unilateral cataract, ASA I or II who undergo cataract excerexis with application of intraocular lens. Patients were randomized into one of two groups to receive sedation with Midazolam 150 µg/ kg in single dose (Control Group) or Dexmedetomidine 0.5 µg/ kg. single dose (Experimental Group) both administered I.V. previous the ocular globe manipulation. Statistical analysis was made by U-Mann-Whitney test.
Results. We get less time and deeper sedation and went up to grade IV of Ramsay scale with Midazolam while in the Dexmetomidine group get sedation later with less deep and only Grade II in scale. Hipoxia and respiratory depression, was grater in Midazolam group. This was observed since infusion, until the first hour of postanesthesic. Haemodinamic behavior in both groups did not show increase of more than 20% in arterial tension and cardiac frequency during infusion, transanesthesic or postanesthesic comparing with baseline.
Conclusion. Sedation with Dexmedetomidine in 0.5 mcgrs per Kg single dose seems to be a viable and safe option in geriatric population carried to cataract surgery with local anesthesia and sedation.
