ABSTRACT
Vaginal enterocele is a rare disease of diverse etiology, which may occur spontaneously or by trauma that is the most frequent and is associated to a history of hysterectomy. Several predisposing factors are considered to be related mainly to hypoestrogenism and weakness of the pelvic floor. Commonly reported symptoms are abdominal pain, vaginal bleeding associated to vaginal foreign body sensation. Diagnosis is clinical noting the presence of small bowel, bladder or other organs in the abdominal cavity. It represents a surgical emergency that warrants reduction of abdominal contents, intestinal resection in case of necrosis and probable enteric anastomosis, pelvic floor repair in a second surgical procedure depending on the conditions of the abdominal cavity; also, provides for the placement of prosthetic material in the vaginal vault. Postoperative complications depend on the patient's medical history and the procedure performed (resection with intestinal anastomosis or bypass enteric). We present the case of a postmenopausal female patient with a history of hysterectomy, patient presented a transvaginal small bowel evisceration after the Pap smear. We reviewed the surgical treatment and complications, considering associated risk factors that increase morbidity and mortality.
