Neurological development during the first year of newborns with birth weight ‹ 1500 g in a third-level institution

ABSTRACT
Introduction: The neurodevelopmental studies reported significant improvement in premature infants with very low birth weight (‹ 1,500 g), but in extremely low birth weight preterm (‹ 1,000 g) reports a great variability of serious sequelae.
Objective: To determine and compare the neurological development of infants born weighing between 1,000-1,499 g compared with those born weighing less than 1,000 g at an institution of tertiary care.
Material and methods: We conducted a cohort study, including newborns weighing less than 1,500 g, measured at 12 months corrected gestational age, born between January 2000 and December 2005. We performed Amiel Tison neurological examination and audiological assessment. We analyzed prenatal and neonatal variables and statistical analysis using Student’s t, chi square and logistic regression to predict the risk of neurological abnormality was established significance with p value ‹ 0.05.
Results: We studied 599 infants, of whom 467 had a weight between 1,000-1,499 g (group 1) and 132 weighing less than 1,000 g (group 2). Group 1 showed 41.1% of normal infants compared with 25.7% in group 2. Mild to moderate abnormalities were similar for both groups. The percentage of severe abnormalities was 13.6% in group 1 compared with 4.1% in group 2. The 1% had sensory hearing loss. Predictor variables for neurological abnormalities were weight less than 1,000 g: RR 2.04, 95% CI (1.25-3.31), male: RR 2.12, 95% CI (1.48-3.03), intraventricular hemorrhage: RR 3.26, 95% CI (1.78-5.98) and sepsis: RR 1.4 (95% CI 1.2-2.15).
Conclusions: Birth weight, male sex, intraventricular hemorrhage and sepsis are risk factors for abnormal development in the first year of life.
