Fertility-sparing treatment in endometrial adenocarcinoma

ABSTRACT
Endometrial cancer is one of the most common gynecological malignancies. The mean age at diagnosis is in the sixth decade of life, up to 25% of endometrial cancer cases occur in premenopausal women, and between 2.5 and 14.4% of the cases involve patients younger than 40 years. The standard treatment for patients with endometrial cancer is hysterectomy and salpingo-oophorectomy as part of the staging surgery. There are several studies that reported various cases of successful conservative treatment, achieving complete response, using hormonal therapy with progesterone in young women with desire of childbearing that meet strict criteria such as endometrioid type of endometrial cancer, low grade disease, no evidence of myometrial or metastatic disease on ultrasound or MRI and accepting and facilitate strict surveillance. The fertility-preserving treatment of endometrial cancer is not standard worldwide, the available evidence suggest that this is a feasible and safe option for a group of selected patients.
