Evaluation of the biochemical marker of myocardial necrosis in acute coronary syndrome

ABSTRACT
Biochemical marker of myocardial necrosis is a useful tool in the diagnosis and prognosis of patients with acute coronary syndrome. Cross-sectional study was conducted in 370 patients of both sexes between 60 and 76 years diagnosed with acute coronary syndrome from the Coronary Care Unit of Clinical and Surgical Hospital “Hermanos Ameijeiras” during the period from January 2008 to January 2009. We evaluated the diagnostic value of two biochemical markers: the mb isoform of the enzyme creatine kinase (CKmb) and troponin T (TnT), as indicators of myocardial necrosis and the relationship with clinical and electrocardiographic manifestations of the patients. It was noted that 95.3% of patients with prolonged angina both markers were positive, of whom 85.7% had ST-segment elevation of more than eight millimeters. 9.6% of patients with ST-segment depression were applied to an early invasive strategy. A quarter of patients diagnosed with myocardial infarction were positive for both biochemical markers demonstrating the diagnostic value of them.
