ABSTRACT
Osteosarcoma is the most common primary malignant bone tumor in children and adolescents. Clinically patients present with pain, swelling and palpable mass. The three most frequent locations are distal femur, proximal tibia and proximal humerus. Before perform a biopsy, we must complete the study of our patient with axial computed tomography of the lesion and chest (to search for lung metastases), magnetic resonance imaging of the primary site including the joint above and below (looking for skip metastases), and a total body radionuclide scan; then, to plane the biopsy. It must be taken in the hospital and by surgeon that will perform the definitive procedure. There are many types of osteosarcoma, but conventional osteosarcoma is diagnosed in 75 to 90% of cases. The treatment includes preoperative (neoadjuvant) chemotherapy and surgery with wide margins. Once you have reconfirmed the diagnosis, verified clear surgical margins, and determined the percentage of tumor necrosis, to continue with adjuvant chemotherapy. When is not possible to preserve the affected limb, radical surgery is indicated.
