Critical incidents and error in medical care. What is the health care provider or the system?

ABSTRACT
The faults committed by the physician may be the result of carelessness in the act, due to negligence, lack of preparation or oversight. Is unlikely to be due to their pride, arrogance or deliberate. To err is human, to hide them is unforgivable and inexcusable not to learn from them. The errors are the result of a series of unfortunate events that were not anticipated, identified or reversed at the time and adversely affecting the quality and safety of care and medical act. This article reviews the substrate of medical error, the conditioning, analysis, impact, implementation of prevention and eradication processes to ensure that all physicians who have the responsibility of caring for sick or direct services or hospitals to make awareness of this new paradigm of modern medicine and the impact of error in the care of the sick.
