Insulin, leptin and insulin-resistance degree in school children with and without obesity

ABSTRACT
Background: Leptin is a hormone of the adipose tissue that reduces the appetite and the corporal fat; the insulin is produced in the pancreatic cells beta, the function is to transport the glucose through the fatty, muscular and hepatic tissues. The excess of fatty tissue is associated to hyperleptinemia, hyperinsulinemia, cardiovascular risks, diabetes and hypertension. There is a relation between infancy obesity and the metabolic alterations that are mentioned above.
Objective: To determine serum insulin and leptin levels and insulin resistance among primary school children with and without obesity.
Participants and method: A transversal study was performed with children from an elementary school (six to nine years old). From the 282 scholars checked only 80 were randomly selected from first and fourth body mass index (BMI) quartile. Data about gender, weight, height, waist circumference, blood pressure, BMI, percentage of BMI, and Z score were recorded. Platelet, leucocytes, hematocrit, hemoglobin, triglycerides, cholesterol, glucose, HOMA (resistance to insulin), insulin and leptin were also measured.
Results: 37 and 43 children were selected of first and fourth quartile, respectively. The levels (mean ± SD) of leptin were: 3.34 ± 2.74 and 26.14 ± 18.17 (
p ‹ 0.001), insulin 10.74 ± 15.31 and 24.37 ± 28.57 (
p = 0.009). Leptin quartiles showed significant differences in weight (
p ‹ 0.001), height (
p ‹ 0.001), waist circumference (
p ‹ 0.001), BMI (
p ‹ 0.001), BMI percentile (
p ‹ 0.001), Z score (
p ‹ 0.001), triglycerides (
p ‹ 0.001), insulin (
p ‹ 0.001), and HOMA (
p ‹ 0.001).
Conclusion: There are differences in metabolic parameters and leptin concentration among groups, leptin could be an early marker of infancy obesity.
