Concentricity effect on acetabular development in developmental dysplasia of the hip

ABSTRACT
The objective of the treatment of hip luxating displasia is to obtain a reduced hip in a stable, congruent and concentric form. This will depend on several factors, such as the patient’s age at the beginning of the treatment, the morphology and the interaction of different joint components. In July 1991, we started a study on the first 25 patients attracted up to that moment and suffering from congenital hip luxation or dislocation according to the diagnosis, without taking into account sex, age or having received no previous treatment. This gave a total of 32 luxated hips. The purpose of our study was to evaluate through X-ray the effect that thigh-bone head exercises over the spontaneous development of insufficient acetabulum. In every case the treatment was carried out according to a logical methodology that, far from being rigid, allows us to adapt the treatment to each patient’s needs, according to the problem stage. The 100% of the hips where a concentric reduction was achieved, according to the Fernández and Sierra bisectrix sign, developed a proper acetabulum covering, being the Wiberg’s CE angle larger than 20°. In addition to this, having suffered from transitory thigh-bone head ischemia or grade I ischemic necrosis did not impede to get a spontaneous development of a good acetabulum covering
