Spinal dural arteriovenous fistula: an underdiagnosed and treatable cause of progressive mielopathy

ABSTRACT
Spinal dural aretriovenous fistula is a treatable cause of mielopathy. It accounts for 70% of spinal arteriovenous malformation, although it remains underdiagnosed in clinical practice.
Clinical cases: we report four patients with progressive weakness of both lower limbs, with diagnosis of spinal dural arteriovenous fistula. In all cases magnetic resonance imaging of the spinal cord showed a mielopathy of central location that extended over several vertebral levels.
Discussion: spinal dural arteriovenous fistula has to be included in differential diagnosis of progressive mielopathy. A high level of suspicion is needed to avoid diagnostic delay.
