Transciliary supraorbital approach for intra-axial lesions

ABSTRACT
Objectives: make a revision of the principal indications of the transciliary supraorbital approach, with a review of this technique, its usefulness and results for the resection of intra-axial brain lesions.
Patients and methods: six adult patients were operated on with this approach. All of them with intra-axial lesion of the frontal lobe, 3 were hematomas, 1 metastases, 1 glioblastoma multiforme and 1 anaplastic astrocytoma.
Results: the procedure was well tolerated by all the patients. In regards of the resolution of each lesion; the hematomas were completely evacuated, as well as the metastases. The cases of Glioblastoma and astrocytoma, had a resection of 80 and 90%, respectively. Only one patient harboring an intracerebral hematoma, died as a result of rebleeding, because of a coagulopathy. Finally the cosmetic results for the patient, as well as for the surgeon, were very acceptable.
Conclusions: the transciliary supraorbiltal approach its a minimally invasive technique that have been used mainly for treating extra-axial lesions of the anterior fossa, sellar region and aneurysms of the circle of Willis. However, there is less information about treating intra-axial lesions but it becomes a very good option, in regards of the resolution of the pathology as well as of the cosmetic results. Also it is very useful to gain experience and familiarization with this technique.
