Suprarenal myelolipoma: review of cases in Mexico

ABSTRACT
Background: Suprarenal myelolipomas are rare, non-functioning and benign tumors that have a good prognosis. They are characterized by the presence of fatty tissue and bone marrow elements of varying degrees of differentiation.
Materials and methods: A review of the Mexican literature of the last 20 years is presented bringing together a total of 18 cases, 4 of which were operated on in the Hospital de Oncologia del Centro Medico Nacional Siglo XXI. Sex, age, affected side, size, presenting symptoms, imaging study findings, treatment and histopathological reports were evaluated.
Results: The men to women ratio was 1.2:1, age range was 30 to 70 years with a mean age of 48 years. The right side was the side most affected with a 2.7:1 ratio in relation to the left side. Tumor size varied from 3 to 22 cm with a mean size of 10.5 cm. The most common symptom preceding diagnosis was lumbar pain which presented in 11 patients. Three patients were asymptomatic. Tomography studies reported densities suggestive of myelolipoma in 10 patients. Partial and total calcification was present in two of those patients. All patients underwent open surgery and one of them also underwent laparoscopy. The main surgical indication was size and pain was size-related.
Conclusions: Even though it has not been possible to establish true incidence in Mexico, it is important to recognize diagnostic and medical or surgical management criteria in order to offer patients the best treatment.
