Endoscopic findings related to laryngopharyngeal reflux in asymptomatic volunteers and symptomatic patients

ABSTRACT
Background: The laryngo-pharyngeal reflux (LPR) is the return of gastric contents towards the larynx and pharynx. It has been questioned by several authors the validity of the endoscopic findings in patients with the diagnosis of LPR, due to similar findings in asymptomatic patients.
Objective: To compare endoscopic findings suggestive of LPR between asymptomatic volunteers and symptomatic patients.
Patients and met hod: We designed a transversal study with asymptomatic volunteers and symptomatic patients. We obtained demographic data and symptoms, and performed a laryngeal endoscopy. The Reflux Symptoms Index (RSI ) and the Reflux Finding Score (RFS) by Belafsky were applied to all patients. The videos were rated by two observers and agreement was determined. Data were analyzed by Chi square and t Student.
Results: We recruited 77 patients, 35 men and 42 women, the mean age was 33.4 years. We formed two groups based on symptoms, 64 asymptomatic volunteers and 13 symptomatic patients. The asymptomatic (54.7% males) were younger. The RSI scored a mean of 3.03 in asymptomatic volunteers and 19.69 in patients (p=0.038). The RFS was 8.50 ± 3.21 vs 10 ± 2.48 (p=0.117) in each group, respectively. In 48 (75%) of the asymptomatic subjects a diagnosis of LPR was established according to RFS. The inter-observer agreement was fair (kappa=0.34).
Conclusion: It is necessary to have a higher limit for the diagnosis of LPR by endoscopy, since endoscopic findings suggestive of LPR are found in the general population.
