Morphology and function in donors and recipients of renal transplantation

ABSTRACT
Introduction. After undergoing a renal transplant, both the recipient and the donors’ kidneys need to adapt to this new situation, in order to maintain their homeostasis. The purpose of this study was to evaluate the changes that occur in the children and their donors after renal transplant.
Method. Renal volume and glomerular filtration were determined for 16 recipients and their donors, in the pre-transplant period and periodically for up to 3 years; the renal reserve was determined once after 6 months of the transplant.
Results. Renal volume increases rapidly in both groups of patients beginning from the first week and up to 3 months thereafter; it then increases slowly in both groups. Glomerular filtration increases in both groups up until 3 months. Afterwards in the donator group it continues to increase at a slower rate, and in the recipient group it diminishes also at a slower rate. The renal reserve is smaller in the recipients, albeit within normal limits.
Conclusions. 1. Both kidneys develop compensating hypertrophy, and increase their glomerular filtration. 2. Renal reserve, although lower in the recipient, is within normal limits, thus indicating that there is no hyperfiltration.
