Angioplasty is the treatment of choice for coarctation of the aorta? A case report and review of literature

ABSTRACT
Background:The coarctation of the aorta is a frequent ailment. It represents from 6% to 8% of patients with congenital cardiopathy. There is still a controversy to choose which is the election treatment in the young patient: the interventionism through stent collocation or the surgical treatment.
Material and Methodology: A 25 years old male patient with diagnosis of coarctation of the aorta was admitted. A medical literature review was made to evaluate benefits and therapeutic results of patients with diagnosis of coarctation of the aorta and treated with the interventionism through stent collocation or the surgical treatment.
Results: The mortality in both procedures is near to 1%. The frequency of restenosis is less in patients with surgical treatment. 90% of the patients who were undergone to interventionism and stent collocation reached optimal conditions related to the improvement of the residual pressure gradient (‹20 mmHg). The patients who were undergone to surgery have a better control in a long term of the blood pressure (83% surgical treatment against 50-75% with stent collocation). Patients who are older than 39 years old have a greater complication incidence when they take a surgical treatment.
Conclusion: The angioplasty with stent collocation offers appropriate benefits to control the pressure gradient in patients with coarctation of the aorta even though it is necessary more studies with a larger number of patients and longer monitoring that allows to value the benefits of this treatment versus the surgical treatment. Even though there is a good response in the control of the blood pressure in patients with coarctation of the aorta treated through angioplasty with stent collocation, the surgical treatment in this aspect offers more benefits.
