Inhibitors of gastric acid secretion for stress ulcer prophylaxis. Appropriate and inappropriate indications and the economic impact on a second level hospital in Mexicali (Mexico)

ABSTRACT
Background: The development of stress ulcer in patients who have had such stressful events it is a well-known entity, nevertheless, from the appearance of the medicaments that increase the gastric pH, the incident of these has diminished. Making clinical use of these inflated estimates has patient care implications and cost implications, because stress ulcer prophylaxis adds to the number of medications given and increases the risk and cost of adverse reactions.
Objective: The present study tries to demonstrate that the overuse of the inhibiting of acid gastric secretion like prevention for stress ulcer in a hospital of The Mexicali’s second level.
Material and method: The study is retrospective, observational, of the patients’ clinical processes of the General Hospital of Mexicali, of patients hospitalized in the services of internal medicine, surgery, emergency room, and intensive therapy in the from June to September, 2009.
Results: 828 patients were included, the percentage that received inadequate therapy they were 73 % of emergency room, 62 % surgery, 42.85 % of medicine hospitalized and 12 % of UCI. The total cost for inadequate prevention was 77,415.52 weight.
Conclusions: The difference in the inadequate prescription on the part of the different services, can be related to the preparation of the personal scholar and assigned.
