False-negative results in breast fine-needle aspiration

ABSTRACT
Objective: To describe the causes and the features of quality of the specimen that cause false negative of the fine needle aspiration biopsy of breast.
Material and method: Sixty cases with false negative diagnosis in fine needle aspiration biopsy of breast were studied. Causes and sample quality directly related to cytologic diagnosis failure were assessed.
Results: In 48.3% of the 60 false negative cases, clinical and mastographic data and presumptive diagnosis were missing. In 53% the elaboration was of poor quality, and 83% were not well fixed. The 33% of the smears showed inadequate cell quantity. Finally in 33% the approaches of malignancy were difficult to identify.
Conclusions: The false negative diagnoses are related to the inadequate sampling, poor quality of the smears and in smaller proportion to an erroneous diagnosis due to the proper nature of the lesion. To optimize the fine needle aspiration biopsy of breast we need: cytological request with clinical and image data, operator’s skills for taking the samples, well preserved smears and enough quantity of cells.
