Gore-Tex® implants placement in patients submitted to increasing rhinoplasty: 12-year-experience

ABSTRACT
Background: Gore-Tex® implants are a recent option in the rhinoplasty performance. Few studies with more than 6 years of following, evaluating esthetic-functional prognosis, have been done.Objective: To report the long-term prognosis (12 years) of patients submitted to increasing rhinoplasty with the use of Gore-Tex® implant.
Patients and methods: Esthetic-functional success and complication incidence (infection, extrusion and migration) associated to surgery type (primary or secondary), suture and approach way were assessed.
Results: Fourty-six patients were included; all of them showed satisfactory esthetic-functional result. Infection incidence was of 6.5% (three patients), all happened in the first 15 days of procedure and with medical resolution. Only one patient required revision surgery, with implant removal and bone graft placement with favorable results. Implant migration happened in 5 cases (10.8%), only in one it was removed and instead shell cartilage was placed. The most important risk factor for this complication was related to primary surgery intervention (OR 8.6, CI 95%, 08-84.9). There were no extrusion cases.
Conclusions Long-term prognosis of patients submitted to rhinoplasty with Gore- Tex® implant was good. The most frequent complication was migration (patients with primary surgery), followed by infection, which was related to the use of chromic catgut suture. It is recommended to place implant with ‹ 5 mm thickness, with good fixation and use of interlaminar prolene suture.
