Barrett’s esophagus and cancer

ABSTRACT
Traditionally Barrett´s esophagus (BE) had been linked with major gastroesophageal reflux disease (ERGE) and with adenocarcinoma. Actually 8-20% of the patients with reflux show Barrett´s features at endoscopy. The risk that this picture turn to malignancy is variable from 0 to 58%.3 The objective of this work is to determine prevalence, clinical, endoscopic and histologic characteristics and frequency of malignant transformation in our population. A retrospective analysis in 520 patients was made: 78 of them showed BE (15% prevalence), 56 males (71.7%) and 22 females (28.2), a relation 3: 1. Age ranged from 9 to 89 years (medium 65 years), 94.8 % had clinical symptoms. Endoscopic findings were ulcers (42.3%) and narrowness (21.7%). Histologically it was found intestinal epithelium (42.3%) and fundic (8.9%). Slight dysplasia in 6.4% and severe dysplasia 1.2%. The relation between the two diseases (adenocarcinoma and BE) was 5.1% two more BE patients developed adenocarcinoma later (6.4%). Follow- up from 2 to 9 years. (medium 11 months). Prevalence of BE in our population ranks at the same level that in all the world. Incidence and prevalence of malignancy is lower, although perhaps it is due to the short period of follow-up.
