Perinatal mortality and maternal morbidity associated with the twin-to-twin transfusion syndrome without intrauterine treatment. The need for a fetal surgery program

ABSTRACT
Aim: To evaluate morbidity and mortality associated with not treated twin-to-twin transfusion syndrome.
Methods: Fifteen (15) monochorionic biamniotic twin pregnancies complicated with twin-to-twin transfusion syndrome (TTTS) were evaluated during the period between 1998-2008. Gestational age at referral, TTTS severity, duration of pregnancy after TTTS diagnosis, fetal mortality and maternal morbidity were registered. In addition, gestational age at delivery, neonatal complications, neonatal survival and long term neurodevelopment were also recorded.
Results: Non treated TTTS was associated with 61% risk of fetal or neonatal mortality. Approximately 16% of survivors presented a long term abnormal neurodevelopment. Only 35% of fetuses were sent home in good conditions and did not develop long term neurological complications. A high percentage (26.6%) of pregnant women presented associated complications as: pregnancy induced hypertension, premature rupture of membranes, chorioamnionitis and placental abruption.
Conclusion: In no treated TTTS, there was a high prevalence of maternal, fetal and neonatal complications. These findings suggest the implementation of intrauterine treatment for this condition.
