Syncope: A combined vision of the prehospital approach

ABSTRACT
Syncope is a sudden loss of consciousness with spontaneous ad integrum recovery. It is a common cause for emergency services consultation. It’s approach should be based on a thourough history and a careful physical exam to define if the symptom implies a higher risk of complications in the patient. Syncope has multiple ethiologies, so it can have different implications in different patients. Prehospital approach should include a complete interrogation about precipitating factors, prodromes, the clinical characteristics of the episode and the patient’s and his family’s history. The physical exam has to search for vital signs in decubitus and orthostatism, search for cardiac murmurs, electrocardiogram, heart monitoring and blood glucose. The most common form of syncope, neurocardiogenic syncope, usually requires hydration and monitoring. Patients with other causes amenable to treat in a prehospital setting include unstable arrhythmias. Most of the other patients require specialized treatment, that is why the emergency medical service is restricted to a prompt recognition of a high risk situation (pulmonary embolism, myocardial infarction or other serious heart or general conditions) and to transport the patient to an adequate facility where the patients will be thouroughly evaluated and treated.
