Clinical profile of dengue in patients from Yucatan. The usefulness of World Health Organization’s (WHO) case definition criteria.
INTRODUCTION. Clinical profile of dengue varies depending on geographic regions and different outbreaks. In Yucatan there is not current information regarding the clinical presentation of this disease. There is not information regarding the percentage of patients with diagnosis of dengue hemorragic fever (DHF) that fulfils the case criteria of WHO.
OBJETIVES. To determine the clinical profile of dengue in patients attended in Mexican Institute of Social Security (IMSS) in Yucatan since January 2004 to December 2006, as well as the percentage of patients that fulfills the diagnostic criteria of WHO for DHF.
PATIENTS AND METHODS. Patients with dengue attended in the IMSS in Yucatan during the mentioned period were included. Epidemiological and clinical studies were performed to each patient. Laboratory data were obtained. Patients were classified as dengue fever (DF), dengue fever with hemorrhagic manifestations (DFHM) and dengue hemorrhagic fever (DHF). Confirmed cases were those with positive specific IgM. Data were analyzed with descriptive and non-parametric statistics.
RESULTS. A total of 209 patients were studied, 130 of which were classified as DF, 17 cases as DFHM and 54 cases as DHF. The most frequent clinical manifestations were fever, headache, arthralgia, myalgia, and retroocular pain. Hepatomegaly and esplenomegaly, torniquet test positive, petechiae and vomiting had a positive statistical association with DHF. Only 22% of patients diagnosed as DHF fulfills all criteria of case definition of the WHO.
CONCLUSIONS. Hepatomegaly and esplenomegaly, torniquet test positive, petechiae and vomiting are data of alert of severe forms of dengue. There is a need of new case definition of DHF to avoid an underregistration of this form of the disease.
