Clinical and epidemiologic findings in 3,183 bacterial meningitis cases. (1980/2007)

ABSTRACT
OBJECTIVE. To describe the clinical, epidemiological and laboratory data of 3,183 cases of bacterial meningitis from January 1980 to December 2007, in the Hospital de Infectología and Hospital de Especialidades at “La Raza” Medical Center of the Instituto Mexicano del Seguro Social.
MATERIAL AND METHODS. Retrospective review of clinical expedients of 3 183 patients with bacterial meningitis confirmed by isolation of the aetiological agent. Statistical analysis of age, sex, aetiological agents, and years of the study were used.
RESULTS. 2,080 patients (65.3%) were males, 1 608 (50.5%) children; global mortality was 22.6%. The most frequent clinical manifestations were fever and meningeal irritation; respiratory tract infections, cerebrospinal fluid shunts, AIDS and malnutrition were the most frequent pathological antecedents. The members of Enterobacteriaceae family were the most frequent aetiological agents isolated, followed by
Staphylococcus spp, Streptococcus spp, Haemophilus influenzae, Mycobacterium spp. and
Cryptococcus spp.
CONCLUSIONS. A general tendency toward lower number of cases was observed, a major frequency of opportunistic pathogens predominantly in AIDS patients and in those with cerebrospinal fluid shunts, was relevant. A decrease in mortality rate was obtained by a better laboratory diagnosis and early clinical-terapeutic intervention.
