Basilar apex aneurism case report and review of pretemporal transzygomatic transcavernous approach

ABSTRACT
Objective and importance: basilar apex aneurism it’s a rare condition; we report the case of a female patient with the diagnosis of basilar apex aneurism treated successfully by a pretemporal transzygomatic transcavernous approach initially reported by Drake and recently retake by Krisht. Clinical presentation: a female patient of 75 years old who develop a subit headache whit temporary loss of consciousness and total neurologic recovery with a Hunt Hess clinical scale of I, image work up showed a basilar apex aneurism with the neck behind the posterior clinoideal process and the tip in touch with it. Intervention: a pretemporal transzygomatic transcavernous approach was done. Clinical evolution was transitory right palpebral ptosis without affection to the eye or pupilar movement, post operatory angiography showed no residual aneurism. We present a review about these approach technical considerations. Conclussion: this case of a basilar apex aneurysm operated by barely used in our field pretemporal transzygomatic transcavernous approach that is less complicated that it seems that gives an excellent visualization of the basilar artery beneath de aneurism neck and allows the mobility of the third cranial nerve whit minimal brain retraction. Actually this approach is becoming a safe and feasible for these lesionsREFERENCES
