Primary melanoma intraventricular

ABSTRACT
Objective and importance: primary melanoma of the CNS it’s a rare condition; we report the case of a primary malignant melanoma of the left trigone that radiographically mimicked a metastatic lesion.
Clinical presentation: a 51-year-old female with antecedents of tabaquism and depression. Two weeks of headache, nausea and vomiting, neurological work-up revealed a large tumor of the left trigone radiographically diagnosed as a metastatic lesion.
Intervention: a transcortical approach by de second temporal circumvolution revealed a pigmented, vascular neoplasm in the left ventricular trigone. The tumor was totally macroscopically resected and histopathological diagnosis of melanoma was made. The patient had no history of cutaneous or ocular melanoma and no other site of disease was ever discovered. The patient had a good neurological outcome no tumoral recurrence was found in a two year follow up.
Conclusion: the case most likely represents primary melanoma of the central nervous system that mimicked a metastatic neoplasm; the differential diagnosis of the ventricular trigone neoplasms is discussed as are the prognostic implications of primary melanoma of the CNS.
