Evaluation of Clinical Aptitude in Family Medicine Residents

ABSTRACT
The purpose of this study was to characterize the clinical aptitude of family physicians in formation. In addition, we explored to what extent years of clinical experience influenced refinement of such an ability. An instrument with real clinical cases was developed. Three cases were included for each of the specialties: Internal Medicine, General Surgery, Pediatrics, and Gynecology and Obstetrics. Through expert rounds, the measurement instrument was validated and included 412 sentences that evaluated nine indicators that make up clinical aptitude. The internal consistency of the instrument was determined with Cronbach alpha proficiency. This reached a value of 0.94. A census was made of the 19 residence headquarters belonging to 14 IMSS delegations. Grade range was 62 to 232, with an average of 149. In global analysis when comparing means determined obtained by residents according to their academic degree, no significant statistical differences were found; in comparison by headquarters, differences were found (p ‹ 0.03). In clinical aptitude scale, the greatest number of residents reached a superficial aptitude (76.3 %) independently of academic degree, 18.3 % a very superficial aptitude, and 5.3 %, an average aptitude. No resident reached a refined or very refined clinical aptitude, and none obtained grades at random. We conclude that clinical aptitude is at low development and does not change with years of experience; it seems that development of clinical abilities is stagnant.
