Symptoms of Imbalance and Vestibular Disease

ABSTRACT
Objective: To assess balance symptoms in patients with and without illusion of movement (vertigo), all with peripheral vestibular disease, which was identified by vestibulo-ocular reflex and in healthy subjects.
Material and methods: To explore balance symptoms in a standardized way, a questionnaire was designed and then administered to 104 adults with and without vestibular disease. Evaluation of the questionnaire was performed using the KR-20 test (0.75), and ROC curves (100 % sensitivity and 98 % specificity). The questionnaire was administered to 44 adult patients with vestibular disease identified by poor vestibulo-ocular reflex in a referral hospital, with and without vertigo, and to 22 healthy subjects.
Results: The frequency of nearly all symptoms was similar in patients with and without vertigo, but different from in healthy subjects (p ‹ 0.01, χ
2). Only falls were more frequent in patients with vertigo than without it (p ‹ 0.01, Mann-Whitney U).
Conclusion: Frequency of imbalance symptoms under specific circumstances is similar in patients suffering from vestibular disease with and without vertigo. When vertigo is absent, an intended evaluation of imbalance symptoms could be useful to evaluate vestibular disease.
