Selective Intraoperative Cholangiography. Factors Related to Choledocholithiasis

ABSTRACT
Objective: To evaluate the authors’ experience with selective intraoperative cholangiography (SIC) in patients undergoing laparoscopic and open cholecystectomy from January 1999 to August 2001.
Materials and Methods: We carried out a prospective study in patients undergoing cholecystectomies. Indications for SIC were: history of jaundice, or laboratory findings (elevation of one or more liver function test results) or observation of a common bile duct enlarged to 6 mm on ultra-sonography or common bile duct stones and intra-operative finding of cystic duct dilatation ≥ 3 mm or common bile duct dilatation ≥ 8 mm.
Results: Three hundred thirty two patients underwent cholecystectomy. In 81 (n = 81) SIC was carried out on patients. Common bile duct stones were discovered in 21 patients, sixteen women and five men. Sensitivity of jaundice was 70 % and of results of liver function elevated to 72 %. Specificity of ultrasonography was 86 %. The positive predictive value of 48 % for jaundice and liver function results test elevated and negative predictive value of jaundice was of 88 %.
Conclusions: SIC during cholecystectomy is a safe and effective method for diagnosis of common bile duct stones based on analysis of five factors studied.
