Behavior of quality indicators for culture in the mycobacterial diagnostic laboratories in Las Tunas province, Cuba

ABSTRACT
Background: The purpose of the quality control of culture is to constantly improve its efficiency and use as a diagnostic and monitoring alternative, thus assuring that the information generated by the laboratory is reliable.
Methods: Quality indicators, as suggested by the Pan-American Health Organization manual for the bacteriological diagnosis of tuberculosis, were calculated in 7,590 sputum samples from patients with positive culture received in two mycobacterial laboratories from Las Tunas province from October 2003 to December 2005.
Results: The contamination rate was 4.5% and 0.3% in the Hygiene, Epidemiology and Microbiology Provincial Center (HEMPC) and the Hygiene, Epidemiology and Microbiology Municipal Center (HEMMC) of Puerto Padre, respectively. The culture contribution to diagnosis was 17.9% in HEMPC and 27.3% in HEMMC. The percentage of patients with positive acid-fast bacilli (AFB) smear and negative culture was 7.7% for HEMPC and null for HEMMC.
Conclusions: Quality indicators of culture in the HEMPC laboratory of Las Tunas province had acceptable values according to international standards, excepting the percentage of AFB-positive/culture negative cases, and the contribution of culture to diagnosis and the contamination rate in the HEMMC laboratory of Puerto Padre. Introduction of these quality indicators of culture to the national network of mycobacterial laboratories will permit to know and to evaluate the results of the laboratory work, thus improving the yield and the quality of tuberculosis diagnosis in Cuba.
