Insulin dependent diabetes mellitus: Down and Somogyi effects in the variations of the glucemia

ABSTRACT
Introduction: Today, the manifestation of insulin dependent diabetes mellitus (IDDM) presents a clear increment tendency. There are two special phenomena: the Dawn and Somogyi effects in the appropriate metabolic control. The purpose of this study was to identify the variations of the glucemia, level of HbA1c and serum lipids. Material and methods: Twenty four young IDDM patients participated; 12 males and 12 female on the basis of a prospective and correlative study. Clinical history and physical exploration, were made trough medical interviews about the personal and family perception of the IDDM. Laboratory exams, corporal composition and glycemic evaluation were obtained 4 times, each day. Results: The self confidence, personal factors and the knowledge about the illness in relatives, were significative correlated (r>0.70; p <0.05) with the age, time of the illness, gender, and the variation of the glycemic levels including the smallest variations of the serum glucose and the Somogyi and Dawn effects. There was an appropriate levels of lipids. A better knowledge of the illness, was associate with a bigger control of illness, smaller glycemic variations, and lower concentrations of HbA1c. Depression features and a less self confidence were present in the older children, mainly in women. Conclusion: The data support the necessity to educate and orientate the infants with IDDM, since early stages of their illness. A better understanding of the illness diminishes the episodes of metabolic discompensation and the serum lipids, and consequently a reduction of complications. Participants in education programs, will have a better control and profile of the illness. Variations of the blood glucose represented in the effects Somogyi and Dawn are a useful indicators to evaluate the impact of treatment and the immediate control
