Oral cavity metastasic carcinoma. Report of two clinical cases

ABSTRACT

Oral cavity cancer constitutes approximately 80% of all malignant tumors representing an impact in human economy. Around 99% of these tumors correspond to carcinoma, which is present in tongue (27%), gingivae (17%), salivary glands (16%), sublingual region (13%) and others (27%). Masculine gender is the most affected by these carcinoma, displaying a 2:1 incidence ratio over female, and frequently the disease is present in male patients over 40 years of age as epidermoid carcinoma (90%). Metastases affecting oral cavity represent 1% of malignant tumor lesions and constitute the primary manifestation in 30% of other malignant tumors, which generally are asymptomatic and cryptic adenocarcinoma. Clinically, they appear as benign or reactive lesions such as granuloma, fibroma and others. We present two clinical cases: the first case is a male patient of 67 years of age, showing a lesion at the left maxillary gingiva, with a vestibular face of 2 cm and a palatine face of 3 cm in length, spanning along the lateral incisor and canine gingiva. Histological report indicates metastasic adenocarcinoma, whose primary lesion corresponds to a bronchogenic carcinoma. The second case is male patient of 47 years of age, diagnosed for gastric carcinoma and presenting two symptomatic nodular lesions at the right maxillary gingival, apparently of four weeks of evolution, which span along the vestibular face of premolars. Histological report indicates metastatic adenocarcinoma. Prognosis of oral cavity metastatic lesions presents an unfavorable sign, since these usually present at advanced stages of the disease in which survival goes from several months up to 2 years.
