Transitory apical balloning associated to stress (Takotsubo syndrome)

ABSTRACT
This is a review and a case report of apical ballooning or Takotsubo syndrome, in a 74 year old female, whose chief complaint started after an emotional event with chest pain, and EKG changes on arrival to the ER, elevated troponin, and without significant occlusive lesions in the coronary angiography, a ventriculography was performed in which we observed apical ballooning of the left ventricle with hyperdynamic movement around the base of the LV, this was documented with a transthoracic echocardiogram and nuclear medicine. The cardiac function was restored 8 weeks after the onset of chest pain. We present a characteristic case of apical ballooning, which was first described in Japan in the 90´s. This entity is characterized by a transient dysfunction of the apical portion of the left ventricle, associated to a stressful situation as a triggering event. The cause for this syndrome is still unknown but there are hypothesis that suggest that is caused by excessive effect of cathecholaminergic discharge.
