Laparoscopic treatment of Spiegel's hernia

ABSTRACT
Introduction: The Spigelian hernia is formed by a protrusion of the peritoneal sac and abdominal viscera or preperitoneal fat through the Spigelian aponeurosis. It’s either acquired or congenital, and consists of 0.12%-2% of all abdominal wall hernias. It is most frequent in the 6
th or 7
th decade of life. The treatment is surgical, and is either conventional or laparoscopic. The laparoscopic treatment is associated with a low recurrence rate.
Case report: 67 year old female with surgical history of laparoscopic fundoplication and appendectomy. She presents with abdominal pain in right lower quadrant of 5 days duration, which is exacerbated upon effort. On physical examination there is a 3 cm para-rectal mass, not reducible that increases in size with Valsalva maneuver. A CT scan was performed where a Spigelian hernia was observed. The defect was corrected laparoscopically with mesh. She was discharged on post op day 1.
Discussion: The Spigelian hernia is rare, and its clinical presentation is varied. Because of this the clinician must have high clinical suspicion to make the diagnosis. Because it is uncommon and difficult to diagnose, we feel that the laparoscopic approach is an important part of the evaluation and treatment.
