Clinical efficacy of a slide-rule for the dosage of propofol compared with diprifusor using bispectral index

ABSTRACT
Objective: To compare the effectiveness of two systems, one manual control system and one automatic control system, both supported by the same pharmacokinetic model of propofol.
Material and methods: We studied 100 patients ASA I/II between 16 and 60 years of age on laparoscopic surgery. Patients were randomly assigned to receive total intravenous anesthesia with an target controlled infusion (TCI) or manually controlled infusion (MCI), based on the pharmacokinetic model described by Marsh. The degree of hypnosis was assessed by BIS to maintain a value between 40 and 60. We evaluated the total consumption of propofol, percentage of time between the target BIS, hemodynamic variables, and anesthetic recovery time.
Results: Both groups had similar clinical and demographic characteristics. The total consumption of propofol was higher in TCI than in MCI (169 ± 48
vs 144 ± 54 µg/kg/min, p ‹ 0.05). At the moment of the loss of consciousness, the induction time and the BIS value were similar in both groups. The percentage of time with target BIS was not different between groups.
Conclusion: Our results show that both methods are equally effective for hypnosis control, when they are implemented using the same pharmacokinetic model and feedbacked using a pharmacodynamic control variant.
