Hand-assisted laparoscopic sigmoidectomy in the management of fistulas secondary to complicated diverticular disease. A safe alternative

ABSTRACT
Objective: To describe the experience of the authors in the use of hand-assisted laparoscopic sigmoidectomy in the management of fistulas secondary to complicated diverticular disease.
Setting: Public and private tertiary medical institutions in Mexico City, Mexico.
Design: Retrospective and observational.
Statistical analysis: Percentages as summary measure for qualitative variables.
Patients and methods: Patients subjected to hand-assisted laparoscopic sigmoidectomty (HALS) complicated diverticular disease. Variables were: age, gener, diagnosis, surgical procedure, surgical time, conversion to open technique, time of oral feeding start, as well as of in-hospital stay, complications, and mortality.
Results: We performed 25 sigmoidectomies and 2 urinary vesicle repairs. Average surgical time was 128 min (95-260) and oral feeding was started at 48 h (24-72). No conversion to open surgery were made. In-hospital stay averaged 5 days (4 to 8). No surgical death occurred.
Conclusion: HALS for fistulas due to complicated diverticular disease is save, retains the benefits of minimally invasive surgery and allows resolving more complicated cases, without conversion.
