ABSTRACT
Massive pulmonary embolism is an often fatal entity in which an early diagnosis and treatment are essential to reduce mortality. The most common criterion to make the diagnosis is arterial hypotension and cardiogenic shock, but the clinical presentation is often variable and non specific. The initial management of every patient in whom we have the clinical suspicion, is the hemodynamic and respiratory support and the assessment to decide the beginning of anticoagulation therapy. The therapeutic alternatives include thrombolysis and embolectomy, there are different parameters that can guide our decision, but it is important to individualize our decision to every case.
