Usefulness of neuromuscular electrical stimulation posterior to botulinum toxin application in children with spastic hemiparesis

ABSTRACT
Introduction: Neuromuscular electrical stimulation and botulinum toxin type A injections have been independently used in the pediatric and adult population for the management of spasticity with varying results. However studies are lacking in the combined use of these two modalities for the treatment of pediatric spasticity.
Objective: To assess the effectivity of combined treatment with neuromuscular electrical stimulation and botulinum toxin in pediatric patients with spasticity secondary to cerebral palsy.
Material and methods: Eight ambulatory pediatrics patients with spastic hemiparesis, without cognitive, visual or conduct alterations. Botulinum toxin was applied on the patients spastic agonist muscles, low frequency bifasic simetric current was applied on antagonist muscles. Patients also received occupational and physical therapy. Movement analysis was done with electromagnetic sensors, manual functionality was assessed with the Jebsen manual function test at the beginning and end of treatment.
Results: Descriptive and comparative statistics were done with the Kruskal-Wallis and Wilcoxon test. Significant improvement was observed in global manual functioning (p = 0.03). Spasticity and range of movement did not vary significantly.
Conclusions: Our study shows that the combined use of low dose BTA injections and NMES at low frequency, improve function of hemiparetic extremity.
