Value of four bleeding risk scales in cardiac surgery with extracorporeal circulation

ABSTRACT
Background: Efforts to reduce the use of homologous blood and blood products began 40 years ago and continues today. Various scales have been developed for predicting bleeding without achieving a single scale.
Method: Prospective, longitudinal, observational comparison, adult patients undergoing cardiac surgery with CPB. Each patient was applied in the preoperative scales Izaguirre, Karkouti, Magovern and Arora. We documented transoperative and postoperative bleeding, was set as the cut point bleeding › 500 mL. Discriminative power of the scales was done with an area under the ROC curve. A value of P £ 0.05 was considered statistically significant.
Results: 150 patients, 56% of females, predicted risk for each scale. Arora: 94% risk is very low, under 5.3%, 0.7% intermediate Magovern: low 45.2%, 53.5% average 1.4% higher Karkouti: under 36%, 57.4% average 6.6% higher; Izaguirre: 90.6% normal, 9.4% higher. The best discriminative power in transoperative Izaguirre and went to the post for Karkouti.
Conclusion: The scale Izaguirre is a useful tool for identifying patients at risk of bleeding in cardiac surgery with extracorporeal circulation.
