Colonization by Candida in patients in critical condition

ABSTRACT
Objective: Evaluate if the colonization by Candida sp. is a factor of badly outcome and early predictor of infection, likewise the associated risk factors in critically ill patients.
Design: Prospective, cross and observational study. Analysis of frequencies was carried out and association with the c
2 or Fisher’s exact test.
Patients: 198 patients were included from the Intensive Care Unit and intermediate care unit at the Hospital Angeles Mocel.
Results: The incidence of colonization was 45.5% and infection of 3.5%. Otherwise colonization increase the stay at the Unit (OR 7.76, 95% CI 4.11-14.63). The multiple-site colonization represent a greater risk of infection (OR 6.52, 95% CI 1.18-36.01) and death (OR 3.19, 95% CI 1.07-9.47). The related risk factors were longer stay at the Unit (OR 7.76, 95% CI 4.11-14.63), broad-spectrum antibiotic (OR 7.00, 95% CI 3.37-14.51), mechanical ventilation (OR 4.41, 95% CI 2.10-9.24) and central venous catheter. (OR 3.98, 95 % CI 2.08-7.62).
Conclusions: Multiple-site colonization predicts the risk of infection and also death. In the other hand the colonization increases the stay at ICU. The risk factors related to colonization were: longer stay at Unit, broad-spectrum antibiotic, mechanical ventilation and central venous catheter.
