Implications and challenges to achieve therapeutic goals in hypertensive patients with metabolic syndrome

ABSTRACT
Hypertension is one of the major cardiovascular risk factors importantly affected by the coexistence with metabolic syndrome. Hypertensive patients with metabolic syndrome usually need significant changes in their life style and may have more difficulty in achieving therapeutic goals. The objective of the present study was to evaluate the treatment and achievement of therapeutic goals in hypertensive patients with and without metabolic syndrome.
Methods: Cross sectional study, 200 consecutive hypertensive patients who attended a Preventive Medical Service were included. Complete medical history, anthropometrical measurements and laboratory studies were obtained.
Results: The prevalence of the metabolic syndrome within the hypertensive population was 41.5%. When comparing previously known hypertensive patients with and without the metabolic syndrome, those with the metabolic syndrome had as expected, more comorbidities. However, there was a reduced number of patients treated with lipid lowering agents (20%) or aspirin (2%) and fewer patients with metabolic syndrome achieved the goals for blood pressure control (37.5 vs 60%, p ‹ 0.001).
Conclusions: Metabolic syndrome is a common problem in the hypertensive population; patients are usually not screened or treated aggressively for the different components of the syndrome. Integral management of the hypertensive patient is urgently needed.
