Estudio comparativo entre las distintas técnicas de disección de las queratoplastías lamelares

ABSTRACT
Purpose: To evaluate which method of dissection (femtosecond laser, mechanical or manual) is the one that offers better visual results, more stability and less complications.
Methods: Review of all the processes of lamellar keratoplasties realized up to the date, and accomplishment of refraction, topography, endothelial count, study of visual quality and optical coherence tomography.
Results: In whole there were analyzed 24 patients (25 eyes) to which there had been realized lamellar keratoplasty previous for keratoconus (84%), ectasia post LASIK (3%) descemetocele (3%), chemical burn (3%), haze post PRK (3%) and radial keratotomy.
The group of the dissection with bubble was the one that presented better visual acuity and minor postoperative spherical equivalent (0.35 and 6.41 respectively), compared with the group of femtosecond (0.24 and 7.49) and that of dissection with microkeratome (0.08 and 8.29). As for the visual quality all the patients had a very important light dispersion (Objective Scatered Index ›10).
Conclusions: The best results as for visual keenness were reached by the group of the bubble being in addition the one that less residual stroma presented, with what it is possible to conclude that to minor residual stroma, the better visual acuity.
