RESUMEN

Propósito: Demostrar la eficacia terapéutica, la tolerancia y la seguridad de la crema de capsaicina al 0.025%, en el control de la neuralgia trigeminal crónica post herpes zoster oftálmico (HZO).
Pacientes y métodos: Se evaluaron 17 pacientes, 11 (64.7%) hombres y 6 (35.3%) mujeres, con promedio de edad al inicio del estudio de 63.8 años (rango 54-76 años). La media del tiempo de seguimiento terapéutico fue de 8.5 meses (rango, 6-14 meses) y el promedio de aplicaciones de capsaicina de 4.6 aplicaciones por semana por paciente (rango, 2-7 aplicaciones por semana).
Resultados: Trece (75.4%) pacientes experimentaron alivio significativo de la neuralgia. El tiempo promedio de alivio parcial o total (puntaje: 0-2) del “tic douloureux” fue de 3.2 semanas (rango, 1-5 semanas). El efecto adverso más comúnmente observado fue el calor (31.5%), seguido por el ardor local (26.3%), experimentados inmediatamente posterior a la aplicación, y el eritema cutáneo (15.7%), no impidiendo la continuación del tratamiento.
Conclusiones: La crema de capsaicina al 0.025% resultó ser una alternativa terapéutica eficaz, tolerable y segura, en el control del dolor neurálgico crónico post HZO.
