Diagnosis of infection for the hepatitis C virus

ABSTRACT
Objective. The aim of this work was to determine the prevalence of infection for the hepatitis C virus (HCV) in blood donors using a screening test by means of immunoassay (ELISA) and suplementary test recombinant immunoblot (RIBA) and the quantification of the viral RNA by means of Reaction chain Polimerase (PCR).
Material and methods. The positive to anti-HCV ELISA blood donors were studied of 1995 at 2003 in the Medical Center of León Guanajuato of Instituto Mexicano del Seguro Social. RIBA and RT-PCR was used to confirm the infection. The risk factors were analyzed in cases confirmed ad not confirmed.
Results. Of 49,272 blood donors, 344 were positive (0.70%); 45 of 112 (40%) they were positive and 67 (60%) negative by RIBA test; 41 of 44 RIBA positive were also positive PCR with load viral mean of 5.11 log copias/mL. The associated risk factors to infection were: alcoholism (OR 7.10, IC 95% 1.51-33.4), relate sexual promiscuous (OR 4.01, IC 95% 0.77-20.8), transfusion (OR 62.9, IC 95% 4.19-882) and hospitalizations (OR 1.72 IC 95% 1.72-28.3).
Conclusions. The infection prevalence for HCV in blood donors by ELISA test is in agreement with that reported at national level; however, 60% of the positive results for ELISA is false (FP); also, a high agreement exists among a result positive RIBA and detection of viral RNA by means of PCR. On the other hand, risk factors exist “non parenteral” that participate in the epidemiology of the infection in our population and whose final mechanism is necessary to elucidate.
