Spinal anesthesia in children. ¿An option that could be retaken?

ABSTRACT
Objectives: Evaluate the characteristics of spinal anesthesia with bupivacaine in children who underwent infradiafragmatic surgeries.
Material and Methods: this is a descriptive, prospective and longitudinal trial. We recruited children less than 15 years ASA 1-3. They where premedicated with atropine 10 µg/kg plus ketamine 1 mg/kg. Spinal anesthesia (L
1-4) was administered with 0.5% hyperbaric bupivacaine (0.01 mg/kg) in all children. The effectiveness was evaluated by parameters such as lack of sensitivity, motor function, hemodynamic stability, sedation and residual analgesia. EKG traces, blood pressure, heart rate and hemoglobin O
2 saturation were evaluated during the study.
Results: 24 children 7.79 ± 4.22 years old were studied. Analgesia was excellent in 87.5% and sufficient in 4.17%, and insufficient in 8.33%. In two patients we found tonic-clonic movements after ketamine administration; one case developed deep hypotension and in another several episodes of desaturation were presented. No other complications were associated with spinal anesthesia.
Conclusions: Spinal anesthesia is a valuable option of great quality and low risk in pediatric surgery.
