Poot kyphosis in children under 5 years

ABSTRACT

A retrospective study was done at the Shriners Hospital for children, Mexico City unit from March 1982 to June 2000, which included 13 patients with a diagnosis of Pott's disease. The mean age of the patients at the time of surgical treatment was 3 years and 7 months, mean follow up was of 18 months. Diagnosis was made with clinical, laboratory and radiologic criteria. Antimicrobial treatment consisted of Streptomycin, Rifampin, INH and Etambuthol for 12 months. The neurologic deficiency of each patient was assessed with Frankel's criteria; with 3 patients type A, 6 type B, 2 type C and 2 type D. Preoperative kyphosis was 12-92º, mean 62.8º. Twelve patients were treated surgically with the following procedures: debridement of the abscess, corpectomy, disquectomy, reduction of the kyphosis with the placement of an autologous rib graft by anterior approach, 11 patients had posterior fusion with Luque rods. All patients improved neurologically by two Frankel's grades; postoperative kyphosis had a mean of 40.6º and 2 patients presented late complications with one nerve root lesion and one protrusion of Luque rod.
