Diffuse alveolar hemorrage and renal acute failure as a manifestation of microscopic polyangiitis. Case report

ABSTRACT

Introduction: The diffuse alveolar hemorrhage (DAH) and the acute renal failure are acute complications of several vasculitides and need a prompt diagnosis and treatment because it can be life threatening. The DAH can be a medical emergency when the respiratory function is affected and when this happen the patient will be required managed in an ICU as a 90% of the cases. The DAH term is a distinct form of intrapulmonary hemorrhage and arises from the bronchial circulation (including alveolar capillaries, arteriolas and venules). The DAH have suspected with the presence of triad: Diffuse alveolar infiltrates (rarely unilateral), hemoptysis and drop in hematocrit.
Objective: Case report of diffuse alveolar hemorrhage (DAH) and acute renal failure associated to microscopic poliangiitis in ICU in male patient.
Design: Case report.
Setting: ICU of Hospital Regional de Alta Especialidad de Oaxaca.
Patient: Male of 48 years old in ICU and diffuse alveolar hemorrhage, acute renal failure and respiratory failure with mechanical ventilation, present skin lesions of vasculitides so initiate protocol with renal and skin biopsy and immunologic test. The report was pauciimmune glomerulonephritis, C-ANCAS positive test and neutrophilic vasculitides in skin. Conclude microscopic poliangiitis, crescentic glomerulonephritis, diffuse alveolar hemorrhage and vasculitides in skin.
